INFORMA's DBK Sectoral Observatory recently published these insights in the 'Image Diagnosis Services' Special Report.
Madrid, 29 October 2026.- According to INFORMA's (Cesce's subsidiary) DBK Sectoral Observatory—a leading provider of business, financial, sectoral, and marketing information in Spain and Portugal—aggregate turnover for companies specialising in image diagnosis stood at €435 million in 2025, after growing 6.1% from 2024. Since the 2021 recovery, the sector has recorded four years of increases of between 5% and 7%.
This dynamism stemmed from the rise in health insurance and the need to turn to private healthcare in view of the public system's management difficulties, enabling operators to expand capacity and invest in cutting-edge technology to improve patient care.
In terms of activity, about 58 million tests were carried out in 2025, up 4% year-over-year, with a total value of €3.55 billion. Public centres generated 65% of the latter figure, while private clinics accounted for 23% and specialised companies for 12%, a segment that continues to gain share.
The expansion of the range of services and the progressive shift in demand towards private providers underpinned this advance by specialised centres, in a context of increasing complexity in the diagnoses requested by doctors.
By modality, X-rays accounted for the bulk of activity, although MRI and CT scans were the most dynamic test types, driven by improved technological provision. The stock of high-technology equipment totalled 3,843 devices in Spain in 2025, with MRI, CT and mammography equipment particularly prominent.
The 17 leading specialised companies, which account for around 80% of business and employ some 5,750 workers, managed a total of 247 sites in September 2026. Of these, 70% were owned, 18% were units in third-party hospitals, and 12% were units in third-party non-hospital centres.
Concentration of supply among specialised companies is high: the top ten operators accounted for 68.5% of turnover for this type of operator in 2025, in a context marked by the entry of investment firms and the integration of operators into large healthcare groups.
